Hospice Care Planner

At MJHS, we are seeking a highly skilled Hospice Care Planner to join our team. As a key member of our business development and marketing team, you will be responsible for evaluating and establishing a plan of care for patients referred for Hospice services.
Key Responsibilities:
- Evaluate patient referrals and develop a plan of care that meets the needs of the patient and family members.
- Facilitate admission of referrals upon discharge from the hospital to a hospice program that meets the needs of the patient and family members.
- Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center.
- Collaborate with the business development and marketing team to develop and implement strategies to increase referrals and admissions to our hospice program.
Requirements:
- Current Registration to practice as a Registered Professional Nurse in NYS.
- Bachelor's Degree preferred.
- Minimum of two years experience in home care nursing with proficiency in physical assessment and interviewing skills.
- Client-service focused with excellent written and verbal communication skills.
- Advanced critical thinking and problem-solving skills, plus the ability to work independently.
Per Diem Rate: $55/hour Weekday, $60/hour Weekend. Saturdays required.
