About the Role

We are seeking a skilled Case Manager to join our team at Middlesex Human Service Agency, Inc. (MHSA). As a Case Manager, you will play a vital role in ensuring the effective delivery of case management, housing search, and advocacy services for clients at our Bristol Lodge Men's and Women's Emergency Shelters.

Key Responsibilities
  • Conduct intake assessments and exit interviews with clients.
  • Develop and implement goal plans for clients, identifying barriers and solutions.
  • Make referrals for health, education, vocational training, employment, counseling, and legal services, providing advocacy as needed.
  • Attend client meetings, case conferences, and group consultations.
  • Mediate conflicts and provide crisis intervention.
  • Ensure compliance with agency policies and applicable regulations.
  • Complete and maintain accurate documentation in the tracking database.
  • Provide administrative support in the absence of the Program Director.
  • Attend regular staff meetings, supervisions, trainings, and conferences.
  • Perform other duties as assigned to ensure program functionality.
Requirements
  • Bachelor's or Associate's Degree preferred, or two (2) years of relevant experience.
  • Knowledge of homelessness issues and housing search.
  • Networking experience with service agencies and community resources.
  • Flexibility, compassion, and a non-judgmental approach to service provision.
  • Excellent interpersonal, written, verbal, telephone, and time management skills.
  • Competent computer skills.
  • Ability to work in a fast-paced, diverse team environment.
  • Reliable attendance and punctuality.
  • Bilingual (English/Spanish) preferred. Cultural competence.
  • Valid driver's license and reliable personal transportation.
  • CPR/AED & First Aid Certification (training provided by MHSA).
