Job Title: Clinical Case Manager and Advocate

We are seeking a highly skilled and compassionate Clinical Case Manager and Advocate to join our team at Association for Autism and Neurodiversity Inc. Job Summary:

The Clinical Case Manager and Advocate will be responsible for building rapport and trusting relationships with a caseload of approximately 12-15 adults, assessing, identifying, and prioritizing clients' needs to inform the development and implementation of a personalized support plan. The successful candidate will also lead direct support professional colleagues in their work supporting shared clients, assist in their professional and personal development, and coordinate community-based visits by support partners and facilitate collaboration with clients' support networks.

Key Responsibilities:
  • Build rapport and trusting relationships with a caseload of approximately 12-15 adults
  • Assess, identify, and prioritize clients' needs to inform the development and implementation of a personalized support plan
  • Lead direct support professional colleagues in their work supporting shared clients
  • Coordinate community-based visits by support partners and facilitate collaboration with clients' support networks
  • Monitor clients' health, safety, and well-being at home, work, and in the community
  • Document progress and update support plans
  • When indicated and with support of clinical leadership, connect clients with crisis management resources
Requirements:
  • Valid driver's license, clean record, and reliable vehicle
  • Master's degree or higher and licensure in occupational therapy, social work, mental health counseling, or related clinical field
  • Professional, academic, and/or personal experience working with neurodiverse adults preferred
  • Competence in supporting clients across various areas of need
  • Interpersonal, organizational, and time management skills
  • Ability to work independently and as part of a team
  • Willingness to participate in ongoing professional development
