About the Role

Care Dimensions is seeking a compassionate and skilled RN Case Manager to join our interdisciplinary team. As a key member of our hospice care team, you will work closely with patients, families, and healthcare professionals to provide high-quality, patient-centered care.

As a RN Case Manager, you will be responsible for coordinating patient care, identifying needs, and providing supportive care in accordance with the attending physician's orders and plan of care. You will work collaboratively with the interdisciplinary team to plan and coordinate care, participate in ongoing comprehensive assessments, and facilitate discussion of issues from your caseload.

We are looking for a self-directed, flexible, and cooperative individual who is able to work effectively within an interdisciplinary team. If you are a registered nurse with a passion for delivering exceptional patient care, we encourage you to apply for this rewarding opportunity.

Key Responsibilities
  • Coordinate patient care and identify needs
