As a RN Case Manager, you will be responsible for coordinating patient care, identifying needs, and providing supportive care in accordance with the attending physician's orders and plan of care. You will work collaboratively with the interdisciplinary team to plan and coordinate care, participate in ongoing comprehensive assessments, and facilitate discussion of issues from your caseload.

Key Responsibilities- Coordinate patient care and identify needs
