As an LPN Intake Coordinator, you will play a critical role in facilitating the transition from facilities to home care or hospice services. The ideal candidate will have at least one year of experience in acute care settings and certified home health agencies. A current license and registration to practice as a Licensed Practical Nurse in New York State are mandatory. **Job Responsibilities:**

  • Serves as a liaison between facilities and home health agency staff regarding patients, policies, and staff.
  • Assists with exploring, identifying, and evaluating present and potential needs and resources in relation to community health programs.
  • Identifies potential referral sources in the community and assists with increasing agency visibility.
  • Identifies the availability of community resources and utilizes those that best meet the needs of patients in the home setting.
  • Evaluates patients referred for home care/hospice care.

**Salary:** The estimated salary for this position is $62,000 - $82,000 per year, based on commensurate experience and qualifications. This figure may vary depending on individual circumstances.
