About the Role

We are seeking a skilled and compassionate Home Health RN Case Manager to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in delivering high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide comprehensive care and case management services to patients, including admission, ongoing care, and follow-up visits.
  • Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Coordinate care with other healthcare providers, including physical therapists, occupational therapists, and home health aides.
  • Ensure accurate and timely documentation of patient care and progress.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Diploma, Associate, or Bachelor Degree in Nursing
  • Minimum of one year of nursing experience, preferably in home health or a related field
  • Strong med surg, ICU, ER, acute experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation
