Job Summary

RiseBoro Community Partnership is seeking a skilled Case Manager to support the smooth and effective functioning of our Adult Basic Education (ABE) and High School Equivalency (HSE) program. The successful candidate will work closely with the Family Development Coach to ensure the program's success and provide supportive services to participants.

Key Responsibilities
  • Assist with registration and enrollment process, including completing Individual Service Plans (ISPs) with students.
  • Provide translation services and digital technical assistance as needed.
  • Conduct daily case management, weekly surveys, monthly interactions, and participant exit interviews, entering data into the funder's Participant Tracking System (PTS) and internal tracking systems.
  • Work with instructors and the Family Development Coach to ensure participants meet identified contextualized literacy goals.
  • Monitor program participants' academic progress and provide supportive services and referrals as needed.
  • Update and maintain the program's Referral Resource Guide and establish community connections to enhance referral services.
  • Provide career development and job search counseling to adult literacy students in individual and group settings.
  • Work with the Family Development Coach to promote and enroll participants in HSE, college, and/or alternative educational pathways.
  • Schedule guest speakers and relevant services for participants.
  • Attend all funder-required professional development trainings and Adult Education Program Improvement Committee meetings.
Requirements
  • Associates degree in Human Services or a related field and 2 years of relevant experience, or a High School diploma (or equivalent) and at least 3 years of relevant experience.
  • Excellent interpersonal, communication, and writing skills.
  • Detail-oriented, organized, and able to multi-task.
  • Proficiency in MS Office Suite.
  • Bilingual (English and Spanish) required.
  • Working knowledge of career counseling and/or adult education preferred.
