About the Role

We are seeking a skilled and compassionate Home Health RN Case Manager to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in delivering high-quality patient care and support to individuals in their homes.

Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Collaborate with physicians to develop and implement individualized care plans, ensuring seamless coordination of care.
  • Coordinate with various healthcare professionals, including physical therapists, occupational therapists, and home health aides, to ensure comprehensive patient care.
  • Communicate effectively with patients, families, and healthcare teams to ensure optimal patient outcomes.
  • Document patient care and progress accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Diploma, Associate, or Bachelor Degree in Nursing
  • A minimum of one year of nursing experience, preferably in home health or a related setting
  • Strong med-surg, ICU, ER, or acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation
