We are seeking a highly skilled and compassionate Social Worker to join our team at Brookside Care Center. As a Social Worker, you will be responsible for providing a full range of case work services to our residents, including counseling, advocacy, and connecting them with community resources.
Key Responsibilities- Complete intake of residents and gather information to develop and write social histories
- Develop and implement social services care plans for each resident and review quarterly
- Coordinate discharge of residents and treatment plans for residents returning home
- Provide counseling and support to residents and their families
- Initiate and set-up service and treatment plans for residents
- Manage day-to-day activities and time management to perform necessary functions in a timely and accurate manner
- Record and maintain accurate records both manually and in the various database systems used
- Make referrals and recommendations to other agencies involved with the residents
- Consult with staff regarding social and emotional factors related to residents' health problems
- Bachelor's degree in Social Work or related field required
- Social Work certification by the State of Wisconsin is preferred
- Two years' experience working with the elderly or handicapped preferred
- Experience with group facilitation and health or social service programs for the elderly populations helpful
- Working knowledge of Microsoft Word and Excel
- Significant experience in working with the elderly population
- Extensive knowledge of community resources and how to access them
- Knowledge of social and psychological factors affecting nursing home residents
- Knowledge of basic human growth and development, behavior, social problems, developmental disabilities, mental illness, interviewing techniques, and various rehabilitation strategies
- Ability to communicate effectively and establish sound working relationships with residents and their families, staff, and outside agencies
- Ability to observe, interpret, and respond to parties' behavior
- Knowledge of state/federal nursing home codes
- Extensive knowledge of community resources
- Ability to maintain confidential and accurate records and prepare reports
- Well-organized, self-motivated, energetic individual
- Ability to advocate for clients' needs and negotiate appropriate and effective services
