Children's Long-Term Support Waiver Service Coordinator Position

We are seeking a skilled social worker/case manager to provide essential services to children with disabilities and their families.
Responsibilities:
- Under the supervision of the CLTS Program Manager, provide social services and case management to an assigned caseload.
- Maintain a deep understanding of the special needs, problems, and goals of clients assigned.
- Stay up-to-date on policies and procedures for funding sources available to the caseload.
- Provide assessment, planning, and monitoring to the caseload according to established guidelines.
- Offer services that include support, locating appropriate resources, collecting and disseminating data and information, and monitoring individual goals.
- Develop an individualized service plan that outlines the desired types and amounts of service a client should receive and the anticipated outcomes.
- Collaborate with other agencies and their personnel to ensure the best services and outcomes for the client.
- Ensure that all participants have input, understand, and support the plan.
- Encourage and advocate for the child and family receiving services to actively participate in the assessment and planning connected with developing a plan of service.
- Participate in agency staffings and attend trainings as assigned in accordance with agency policy and at the direction of the RC Coordinator.
- Prepare correspondence, reports, and maintain records as required by agency policy.
- Conduct all activities in keeping with accepted standards of ethics for social workers/case managers.
- Perform other duties as assigned by the Director of Community Resource Center and/or the CLTS Program Manager.
Qualifications:
- Bachelor's degree in human services or a related field is required.
- Ability to effectively communicate, both written and verbally, in the English language.
- Must have reliable transportation and a valid driver's license with a good driving record.
- Must carry own auto insurance with at least 100,000/300,000/100,000 coverage.
- Ability to maintain records and document case notes daily.
- Ability to give and accept feedback as a part of a team.
- Ability to traverse within the workspace and community frequently throughout the day.
- Be willing to learn and work with the treatment philosophies.
- Bilingual in Spanish is a plus.
- Experience working with children with disabilities and their families is a plus.
