About the Role:

We are seeking a skilled Clinical Care Coordination Manager to join our team. This role will oversee the coordination of patient care services, ensuring that our patients receive the highest level of care and support. The ideal candidate will have experience in home healthcare coordination, with a strong focus on quality improvement and patient satisfaction.

Responsibilities:

  • Coordinate patient care services, including assessments, care planning, and coordination of services
  • Collaborate with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes
  • Develop and implement quality improvement initiatives to enhance patient satisfaction and outcomes
  • Manage budgets and resources to ensure efficient use of company funds
  • Provide coaching and guidance to staff members to promote professional growth and development

Requirements:

  • Registered nurse with current licensure to practice nursing in the practicing state
  • Minimum 2 years of experience in home healthcare coordination or a related field
  • Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
