Program Services Coordinator

We are seeking a skilled Program Services Coordinator to join our team at Community Access Unlimited. In this role, you will be responsible for ensuring the coordination of quality medical services/care for members.

Responsibilities:
As a Program Services Coordinator, your key responsibilities will include:
• Ensuring a high level of quality assurance regarding members' medical needs, follow-up, etc.
• Completing medical audits on MAR's, medical files, and medication boxes appropriately and ensuring staff follow up on appointments and needs as expected.
• Maintaining effective member/program relations
• Conducting basic member oversight regarding medical issues and situations and directing further needs to the program supervisor/RN/DON
• Conducting staff training on member medical needs, etc. (such as specialized diets/equipment/etc)
• Conducting training with members on medical issues and situations
• Thoroughly documenting all member medical/health issues and situations
• Maintaining program operations and facilitating program development along with program supervisors
• Cooperating with Agency, legal authorities, and Department of Human Services or any part or successor thereof in any inspection or investigation
• Ensuring compliance with all required state and federally mandated background and exclusion checks including but not limited to criminal history background investigation (fingerprinting), Central Registry, Child Abuse Record Information (CARI), and motor vehicle checks.
• Ensuring compliance with wearing a mask when mandated by State, Federal, and Licensing authorities during a public health crisis/pandemic.
• All other nursing-related duties as assigned
Shift and Schedule:
This position requires flexibility in terms of scheduling, including evenings and weekends, as required by program needs. Substitutes work on an as-needed basis, but a minimum of 30 hours per month. Full-time staff work 40 hours per week.
Salary and Benefits:
The estimated salary for this position is $60,000 - $80,000 per year, depending on experience and qualifications. Our organization offers a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
Requirements:
To be considered for this role, you must have a degree in Nursing (LPN) and at least one year of experience working with individuals with developmental disabilities. You should also possess excellent communication and organizational skills, as well as the ability to work effectively in a team environment.
