Job SummaryWe are seeking a Service Coordinator to join our team. As a Service Coordinator, you will be responsible for providing linkages with community resources, daily living skills training, counseling, and support to our clients.
Key Responsibilities- Facilitate clients' linkage with community services, including case management, clinical services, and day programs.
- Develop and implement quarterly goal plans, functional assessments, and goal plan reviews.
- Provide weekly meetings with clients to develop skills and review progress.
- Document all client interactions and progress in a timely and accurate manner.
- Provide individual counseling and support services to clients.
- Assist clients with housing-related issues, including maintenance and repairs.
- Provide daily living skills training, including medication management, cleaning, and money management.
- Perform monthly fire safety inspections.
- Attend weekly supervision and staff meetings.
- Remain current with all agency-required compliance documents and trainings.
To be successful in this role, you will need:
- A Bachelor's degree in Psychology, Rehabilitation, Social Work, or a related mental health discipline.
- At least 2 years of experience working in a similar setting and/or with individuals with mental illnesses.
- Excellent verbal and written communication skills.
- The ability to make sound judgments regarding client care.
