We are seeking a highly motivated and compassionate Care Coordinator to join our team at HireLiving, Inc. as a Connections Specialist. In this role, you will work closely with our emergency department staff to provide support to youth and their families, connecting them to community resources and therapeutic services.
Key Responsibilities- Provide support to consumers and family members during emergency room visits, gathering demographic and clinical information and conducting interviews to assess youth's level of need.
- Develop and maintain a network of community contacts, facilitating referrals and linkages to community services and serving as a liaison to local human services agencies.
- Write concise and accurate observation notes for consumer records, documenting needs and interactions in a timely manner.
- Notify Children's staff of unusual incidents or observations and participate in periodic team meetings to provide input on workflow, issues, successes, and program improvement.
- Bachelor's degree in a social service-related field from an accredited college or university and one year of experience in a social service-related field, or high school diploma or GED and three years of experience in a social services-related position.
- Preferred qualifications include a bachelor's degree in a related field and two years of professional experience in a human services field, knowledge of community resources, case management and crisis intervention experience, and comfort working with a diverse range of consumers.
