Job Summary

We are seeking a dedicated and detail-oriented Service Coordinator/Case Manager to join our team. The successful candidate will provide overall service coordination, advocacy, and services arrangement for consumers and families. This includes developing and implementing Individual Program Plans (IPPs), Person-Centered Plans (PCPs), progress reports, and annual reviews for consumers in traditional and/or Self-Determination Programs.

Key Responsibilities
  • Provide service coordination, advocacy, and services arrangement for consumers and families.
  • Develop and implement Individual Program Plans (IPPs) and Person-Centered Plans (PCPs).
  • Conduct progress reports and annual reviews for consumers.
  • Seek community resources and provide information and referral services.
  • Initiate the purchase of vendored services and monitor service delivery.
  • Stay up-to-date with state and federal regulations, agency policies, and procedures related to consumer services.
Requirements
  • Bachelor's degree in human services or a related field, such as social work, psychology, sociology, guidance and counseling, or child development.
  • At least two years of experience working with individuals with developmental disabilities or in a related field.
  • Bilingual language fluency (Spanish/English) is highly desirable.
  • Strong organizational and time management skills.
  • Excellent written and verbal communication skills.
  • Ability to provide timely and responsive customer service.
  • Valid driver's license and reliable transportation.

Salary Range

The salary range for this position is $21.63 to $30.44 per hour, with a starting salary range of $21.63 to $25.04 per hour.
