Position Overview:


Become an integral part of a prestigious academic healthcare institution, The University of Chicago Medicine Ingalls Memorial, as a Clinical Pharmacist Specialist focusing on General/Internal Medicine within the Inpatient Pharmacy.

In this capacity, you will take on the responsibility and accountability for ensuring the safe, effective, and timely delivery of medication therapy.

Your role will involve supporting both centralized and decentralized medication-use systems, while providing optimal medication therapy to patients experiencing a diverse array of health conditions.

As a Clinical Pharmacist Specialist, you will deliver direct patient-centered care and integrated pharmacy operational services in a collaborative practice environment alongside physicians, nurses, and other healthcare professionals.

Key Responsibilities:


Clinical Pharmacist Specialists primarily focus on supporting the general medicine, medicine-surgery, and telemetry inpatient care units, delivering medication therapy management within these areas and ensuring that pharmaceutical care programs are effectively integrated throughout the organization.

In these clinical roles, you will engage in all necessary components of the medication-use system while providing comprehensive and tailored pharmaceutical care to patients in your designated areas.

Pharmaceutical care services encompass, but are not limited to:

  • Assessing patient needs and incorporating age and disease-specific factors into drug therapy and patient education.
  • Adjusting care based on patient responses and providing clinical interventions to identify, mitigate, and prevent medication-related adverse events.
Essential Functions:


Facilitate pharmaceutical care services, direct patient care programs, and medication utilization systems within assigned services and care areas (primarily inpatient settings) to ensure drug utilization activities align with patient care needs, evidence-based best practices, and regulatory standards.


Proactively identify practice-related issues requiring evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as necessary to enhance practice.

Develop and oversee policies and procedures for drug procurement, usage, distribution, and control.


Maintain proficiency in decentralized pharmacy services and clinical pharmacy programs, prioritizing the delivery of patient-centered care.


Collaborate as an active member of a multidisciplinary team, working closely with healthcare providers in decentralized patient care settings.


Efficiently maintain patient information and interpret patients' healthcare needs, including problem lists and prioritized pharmacist interventions.


Collect and document accurate patient medication histories, reconciling discrepancies against orders.


Identify high-risk patients and implement interventions aimed at improving quality and safety.


Make appropriate evidence-based, patient-centered medication recommendations.


Implement, evaluate, and refine pharmaceutical care plans based on patient progress and evolving medication usage goals, including drug dosing and monitoring.


Provide written documentation in the patient's medical record as requested or directed.


Participate in the management of medical emergencies and provide discharge medication review, reconciliation, and counseling as appropriate.


Deliver evidence-based medication education to physicians, nurses, and other healthcare professionals.


Educate patients on the safe and effective use of medications, potential side effects, and the role of prescribed medications in their overall therapeutic care plan.


Conduct critical patient monitoring and review patient profiles/charts to identify, prevent, or mitigate drug-related issues, including medication errors or adverse drug reactions.


Maintain Medical Staff credentials as required for practice assignments.

Qualifications:


Graduate of an ACPE accredited College of Pharmacy.


Doctor of Pharmacy or other advanced degree required.


Completion of ASHP accredited PGY-1 Pharmacy Residency with inpatient clinical experience or PGY-2 Specialty Pharmacy Residency in Internal Medicine or Cardiology preferred.


Clinical pharmacy experience (2-4 years) in a hospital setting preferred.


Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and evidence-based medicine relevant to clinical assignments.
