We are seeking a Clinical Pharmacist Specialist to enhance our teams in the Infusion and General Medicine/Internal Medicine departments. In this pivotal role, you will take charge of ensuring safe, effective, and timely medication therapy for our patients.

Role Responsibilities

The General Medicine Pharmacist focuses on delivering specialized support for inpatient care areas, including general medicine, medicine-surgery, and telemetry floors. You will manage medication therapy within these settings and ensure that pharmaceutical care programs are seamlessly integrated throughout the institution. This role involves comprehensive pharmaceutical care, including assessing patient needs, tailoring drug therapy to specific age and disease characteristics, and providing education to patients while making clinical interventions to prevent medication-related issues.

The Infusion Pharmacist is expected to demonstrate expertise in caring for a diverse patient population, including pediatric, adolescent, adult, and geriatric patients. You will provide pharmaceutical care that aligns with departmental and hospital standards, ensuring patient safety and minimizing the risk of healthcare errors. Adherence to corporate compliance requirements is essential, and you will communicate any arising issues to the relevant parties.

Key Responsibilities

  • Facilitate pharmaceutical care services and medication utilization systems in assigned areas, primarily focusing on the Emergency Department, ensuring alignment with patient care needs and regulatory standards.
  • Identify and evaluate practice-related issues, facilitating clinical research, quality improvement initiatives, or healthcare provider education as necessary.
  • Develop and oversee policies for drug purchasing, usage, distribution, and control.
  • Maintain proficiency in decentralized pharmacy services and clinical pharmacy programs.
  • Prioritize patient-centered care in all activities.
  • Collaborate as an active member of a multidisciplinary team within decentralized patient care areas.
  • Accurately maintain patient information and interpret healthcare needs, including identifying high-risk patients and implementing safety interventions.
  • Conduct thorough medication history collections and reconcile discrepancies.
  • Provide evidence-based medication recommendations and develop pharmaceutical care plans based on patient progress.
  • Participate in managing medical emergencies and provide discharge medication reviews and counseling.
  • Educate patients on the safe and effective use of medications and possible side effects.
  • Monitor patient profiles to identify and prevent drug-related problems.
  • Document and address issues with medication orders or therapeutic regimens.
  • Maintain appropriate Medical Staff credentials for practice assignments.

Qualifications

  • Graduation from an ACPE accredited College of Pharmacy.
  • Doctor of Pharmacy or equivalent advanced degree required.
  • Completion of an ASHP accredited PGY-1 Pharmacy Residency with experience in an Emergency Department setting; PGY-2 Specialty Pharmacy Residency in Emergency or Critical Care Medicine preferred.
  • 2-4 years of clinical pharmacy experience in a hospital environment preferred.
  • Extensive knowledge in advanced pharmacy practice and specialty medication therapy management.
