We are seeking a dedicated Clinical Pharmacist Specialist to enhance our services in the Infusion and General Medicine/Internal Medicine departments. In this pivotal role, you will be accountable for delivering safe, effective, and timely medication therapy to our patients.
Role Responsibilities:
The General Medicine Pharmacist will focus on supporting inpatient care units, including general medicine, medicine-surgery, and telemetry, ensuring optimal medication therapy management. You will engage in all facets of the medication-use process, providing tailored pharmaceutical care to patients in your designated areas. Your responsibilities will include:
- Assessing patient needs and tailoring drug therapy to individual characteristics.
- Implementing clinical interventions to prevent medication-related issues.
- Collaborating with a multidisciplinary team to ensure comprehensive patient care.
The Infusion Pharmacist will utilize their expertise to care for a diverse patient population, including pediatric, adolescent, adult, and geriatric patients. You will be responsible for:
- Delivering pharmaceutical care that aligns with our institutional mission and professional standards.
- Ensuring patient safety and minimizing the risk of healthcare errors.
- Maintaining compliance with corporate policies and procedures.
Key Functions:
- Facilitating medication management programs within assigned care areas.
- Identifying and addressing practice-related challenges through clinical research and quality improvement initiatives.
- Overseeing policies related to drug purchasing, distribution, and control.
- Maintaining expertise in decentralized pharmacy services.
- Prioritizing patient-centered care in all interactions.
Qualifications:
- Graduation from an ACPE accredited College of Pharmacy.
- Doctor of Pharmacy or equivalent advanced degree.
- Completion of an ASHP accredited PGY-1 Pharmacy Residency, with experience in an Emergency Department setting preferred.
- 2-4 years of clinical pharmacy experience in a hospital environment.
- In-depth knowledge of advanced pharmacy practice and specialty medication management.
