This role encompasses a focus on two key areas: Infusion and General Medicine/Internal Medicine.
Role Responsibilities:
In this position, you will be entrusted with the responsibility of ensuring safe, effective, and timely medication therapy for our patients. You will engage in various departmental assignments, providing support for both centralized and decentralized medication-use systems. Your expertise will be essential in delivering optimal medication therapy across a diverse range of medical conditions.
General Medicine Focus:
As a specialist in General Medicine, your primary focus will be on delivering comprehensive medication therapy management within the inpatient care settings. You will ensure that pharmaceutical care programs are seamlessly integrated throughout the institution. Your clinical acumen will allow you to participate in all facets of the medication-use system, offering individualized pharmaceutical care tailored to the unique needs of each patient.
Infusion Pharmacy Expertise:
The Infusion Pharmacist role requires proficiency in managing the medication needs of a varied patient demographic, including pediatric, adolescent, adult, and geriatric populations. You will be responsible for upholding the highest standards of pharmaceutical care in alignment with departmental and hospital policies, ensuring patient safety and minimizing the risk of healthcare errors.
Key Responsibilities Include:
- Facilitating medication therapy services and patient care programs within assigned areas.
- Identifying and addressing practice-related issues, leading clinical research projects and quality improvement initiatives.
- Developing and overseeing policies related to drug purchasing, usage, and distribution.
- Maintaining proficiency in decentralized pharmacy services and clinical pharmacy programs.
- Collaborating with multidisciplinary teams to enhance patient-centered care.
- Conducting thorough patient medication histories and reconciling discrepancies.
- Implementing evidence-based medication recommendations and monitoring patient progress.
- Providing education to healthcare professionals and patients regarding medication safety and efficacy.
Qualifications:
To be considered for this role, candidates must possess the following:
- A degree from an ACPE accredited College of Pharmacy.
- A Doctor of Pharmacy or equivalent advanced degree.
- Completion of an ASHP accredited PGY-1 Pharmacy Residency, with experience in an inpatient setting.
- Preferred completion of a PGY-2 Specialty Pharmacy Residency in Emergency or Critical Care Medicine.
- 2-4 years of clinical pharmacy experience in a hospital environment.
- In-depth knowledge of advanced pharmacy practice and specialty medication management.
Work Environment:
This is a full-time position with rotating shifts in the Infusion and Internal Medicine departments. The role is based within the inpatient pharmacy unit.
