Clinical Case Manager

We are seeking a skilled Clinical Case Manager to join our healthcare team at Comprehensive Community Action. As a clinical case manager, you will work closely with patients, families, and healthcare providers to ensure seamless care transitions and optimal health outcomes.

About the Role:
  • Conduct comprehensive assessments to identify patient needs and develop individualized care plans.
  • Coordinate care across settings, including hospitals, clinics, and home-based care.
  • Collaborate with multidisciplinary teams to ensure patient-centered care and improved health outcomes.

Requirements:

  • Current licensure as a Registered Nurse (RN)
  • 2-4 years of experience in healthcare, preferably in case management or a related field
  • Excellent communication and organizational skills

Salary: $74,000 per year, increasing with experience.
