POSITION OVERVIEW

The Licensed Practical Nurse (LPN) plays a crucial role in delivering care and treatment in line with physician directives. Working collaboratively within a team, the LPN operates within the scope of practice as defined by state regulations. Under the supervision of physicians and registered nurses (RNs), the LPN is responsible for providing direct nursing care to residents and overseeing daily nursing operations, ensuring compliance with current federal, state, and local standards that govern long-term care and skilled nursing facilities. Your commitment to maintaining the highest quality of care for our residents is paramount.

KEY RESPONSIBILITIES

  • Conducts patient assessments through physical examinations and relevant diagnostic tests to evaluate health status.
  • Administers prescribed medications and treatments.
  • Engages in the care planning process.
  • Supervises the activities of LPNs and certified nursing assistants.
  • Communicates effectively with physicians regarding any changes in residents' conditions and diagnostic test outcomes.
  • Documents assessments and care in accordance with established standards and company policies.
  • Provides education to patients and their families on health-related matters.
  • Completes necessary documentation in line with company policies and state or federal regulations.

QUALIFICATIONS

  • Proficient in computer systems, including Point Click Care.
  • Familiarity with nursing and medical practices, procedures, and standards of care.
  • Graduate from an accredited nursing program.
  • Possession of a valid state license as an LPN.
  • Current CPR certification.

EXPERIENCE

Preferred: One year of experience in a long-term care setting.

LICENSE REQUIREMENTS

Must hold a current, unencumbered, active license to practice as an LPN in Colorado, along with CPR certification.
