POSITION OVERVIEW
The Licensed Practical Nurse (LPN) plays a crucial role in delivering patient care and treatment as per physician directives. Collaborating within a multidisciplinary team, the LPN operates within the scope of practice defined by state regulations. Under the supervision of physicians and registered nurses (RNs), the LPN is responsible for providing direct nursing care to residents while overseeing daily nursing operations in compliance with current federal, state, and local standards governing long-term care and skilled nursing facilities. Your role is vital in ensuring that our residents receive the highest quality of care.
KEY RESPONSIBILITIES
- Conducts patient assessments through physical examinations and relevant diagnostic tests to evaluate health status.
- Administers prescribed medications and treatments effectively.
- Engages in the care planning process to enhance patient outcomes.
- Supervises other LPNs and certified nursing assistants to ensure quality care delivery.
- Communicates promptly with physicians regarding any changes in resident conditions or test results.
- Maintains accurate documentation of assessments and care in accordance with care standards and company policies.
- Provides education to patients and their families on various health-related topics.
- Completes necessary documentation as per company policy and state or federal regulations.
QUALIFICATIONS AND SKILLS
- Proficient in computer systems, including Point Click Care.
- Strong understanding of nursing and medical practices, procedures, and standards of care.
- Graduate from an accredited nursing program.
- Possession of a valid state license as an LPN.
- Current CPR certification is required.
EXPERIENCE
Preferred: One year of experience in a long-term care setting.
LICENSE REQUIREMENTS
Must hold a current, unencumbered, active license to practice as an LPN in Colorado, along with CPR certification.
