Job Summary:

This full-time position involves conducting home-based assessments, developing treatment plans, and collaborating with a multidisciplinary team to deliver effective family preservation services. The successful candidate will have excellent communication and interpersonal skills, a strong commitment to delivering culturally responsive services, and a passion for working with diverse populations.

Responsibilities:

  • Conduct comprehensive assessments of clients and their families to identify areas for improvement.
  • Develop and implement personalized treatment plans that promote healthy coping mechanisms and parenting skills.
  • Collaborate with a multidisciplinary team to ensure seamless delivery of services.
  • Maintain accurate records and documentation as per Medicaid standards.
  • Make referrals for specialized evaluations and treatment when necessary.
  • Participate in treatment team meetings to ensure continuity of care.

Requirements and Qualifications:

  • Masters degree in social work or related field required.
  • Licenses and certifications (LHMC, LICSW, LMFT, etc.) as per Rhode Island Department of Health requirements.
  • A minimum of 6 months' experience working with children, adolescents, and/or families.
  • Strong organizational, critical thinking, and time-management skills.
  • Bilingual in Spanish preferred.

Salary and Benefits:

The annual salary for this position is approximately $60,000 - $70,000, depending on experience, plus benefits including health insurance, retirement plan, and paid time off.
