Position Overview

ROLE SUMMARY

The Geriatric Nurse Practitioner is responsible for providing comprehensive healthcare services to residents in a hospice setting. This role involves conducting thorough physical assessments and implementing preventive health strategies in accordance with the directives of the Medical Director.

KEY RESPONSIBILITIES

  • Conducts regular visits to designated units, performing detailed physical examinations and health assessments.
  • Orders and interprets diagnostic tests, such as laboratory work and imaging studies, to evaluate patients' health conditions.
  • Provides guidance to residents regarding their diet, activities, medications, and overall health status.
  • Manages medication protocols, ensuring safe and effective treatment plans are followed.
  • Integrates findings from health histories and examinations to create comprehensive care plans tailored to individual patient needs.
  • Assesses and prioritizes care challenges, taking necessary medical actions to address them.
  • Delivers nursing care as required, assisting residents with daily living activities and responding to their needs.
  • Recognizes personal professional boundaries and appropriately refers patients to the Medical Director or other specialists when necessary.
  • Collaborates with the Medical Director and interdisciplinary teams to develop and refine patient care strategies.
  • Prepares and submits medical review documentation in compliance with regulatory standards.
  • Engages in Resident Care Assessment Team activities and other committees as directed by the Medical Director.
  • Maintains effective communication with nursing leadership and contributes to nursing programs as appropriate.
  • Involves in the education of PA, DO, and NP students, and collaborates with Geriatric fellows.
  • Commits to ongoing professional development through research, training, and educational opportunities.
  • Prepares and presents reports and communications to staff, residents, families, and the community.
  • Facilitates communication among department staff, nursing services, physicians, and family members.
  • Assumes on-call duties as designated by the Medical Director.
  • Actively participates in initiatives to optimize antibiotic usage, reviewing data to ensure adherence to best practices.

QUALIFICATIONS

Master's degree in Advanced Practical Nursing from an accredited institution. Must be a graduate of an accredited nursing program as a Registered Nurse with a minimum of two years of experience in geriatric care.

LANGUAGE PROFICIENCY

Ability to comprehend and interpret scientific literature, financial documents, and legal materials. Capable of addressing inquiries from clients and regulatory bodies effectively. Proficient in drafting articles and presentations that meet established guidelines.

MATHEMATICAL ABILITY

Competence in applying mathematical concepts such as statistics and ratios to real-world scenarios.

ANALYTICAL SKILLS

Ability to identify problems, gather data, and draw logical conclusions. Skilled in interpreting complex technical instructions and managing multiple variables.

CERTIFICATIONS

  • Advanced Registered Geriatric Nurse Practitioner certification in Florida.
  • Valid Florida Driver's License.
  • Florida Medicare Number.
  • Basic Life Support Certification.

PHYSICAL REQUIREMENTS

The physical demands outlined are representative of those required to perform the essential functions of this role. Reasonable accommodations may be made for individuals with disabilities. Occasionally, it may involve sitting, reaching, and assisting with the positioning of residents weighing up to 200 pounds. Specific vision capabilities required include close and distance vision.
