Job Summary

We are seeking a skilled and compassionate Nurse Care Specialist to join our team. As a key member of our nursing staff, you will be responsible for providing high-quality care to our residents, working collaboratively with our interdisciplinary team to develop and implement individualized care plans, and promoting a culture of respect, empathy, and compassion.

Responsibilities
  • Provide direct care to residents, including administering medications, performing vital signs, and assisting with daily living activities.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Conduct regular assessments of residents' physical and emotional needs, and make recommendations for care and services.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Provide education and support to residents and their families on health-related topics and care management.
  • Maintain accurate and timely documentation of resident care and services.
  • Supervise and mentor Certified Nursing Assistants (CNAs) and other support staff.
  • Participate in on-call rotations and provide emergency care as needed.
Requirements
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of Florida.
  • At least one year of experience in geriatric nursing or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Knowledge of geriatric care principles and practices, including falls prevention, wound care, and pain management.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
