Senior Medical Care Specialist

We are seeking a skilled Nurse Practitioner to join our team at Abode Care Partners.

About the RoleThis Senior Medical Care Specialist will provide high-quality, patient-centered care to individuals from post-hospitalization to home in various settings. The ideal candidate will have excellent clinical skills, strong communication abilities, and a passion for delivering compassionate care.
Key Responsibilities:
- Provide evidence-based medical care to patients with chronic and urgent episodic illnesses, minor injuries, and other health concerns.
- Order and interpret laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals to ensure comprehensive, interdisciplinary care.
- Maintain accurate and up-to-date patient medical records.
To be successful in this role, you will need:
- A current state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- A Master's degree in Nursing (MSN).
- At least five years of nursing experience, with two years of nurse practitioner experience preferred.
- Excellent interpersonal communication skills and time management abilities.
- Proficiency in medical instruments and equipment, electronic medical record systems, and Microsoft Office applications.
