Job Summary

Bridgewell is seeking a dedicated and compassionate Direct Support Professional 3 to provide support and assistance in Level 3 residential programs. As a key member of our team, you will work closely with individuals who require daily intervention, supervision, and skills training in activities of daily living while managing within a home environment and in the community.

Key Responsibilities
  1. Provide support and assist individuals with skills training in all activities of daily living, including personal care, meal preparation, and household management.
  2. Create opportunities and provide support to individuals to make decisions and develop their skills, fostering independence and self-advocacy.
  3. Ensure services are delivered according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards, as well as Bridgewell policies and procedures.
  4. Provide support and assist individuals with all medical, clinical, and financial needs, including transportation to medical appointments and activities.
  5. Maintain accurate and detailed documentation according to regulatory standards and Bridgewell policies.
  6. Develop positive relationships with guardians, family members, and other stakeholders, ensuring effective communication and collaboration.
Requirements
  1. High school diploma or GED required; Associate's degree or DSP certification preferred.
  2. Minimum 1 year of experience working with people with developmental or psychiatric disabilities.
  3. Excellent written and oral communication skills, with ability to write clear and articulate reports.
  4. Valid driver's license and minimum 1 year of driving experience.
  5. Computer literacy, including proficiency in Microsoft products and web-based applications.
