Job Title: Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at Smarter HR Solutions LLC. As a Case Manager, you will play a critical role in addressing the housing, infrastructure, and public service needs of the low to moderate-income community in Harris County.

Job Summary:

The Case Manager will assist clients with a range of concerns, including housing stability needs, financial assistance, location of shelter or housing, job preparedness, and more. This role requires strong critical thinking skills, knowledge of HUD grants, and the ability to work with vulnerable populations.

Responsibilities:
  • Conduct in-person interviews to assess client needs and develop comprehensive plans.
  • Secure information about clients' medical and social factors that contribute to their situation.
  • Assist clients with issues such as neglect, abuse, domestic violence, mental health, hopelessness, chemical addiction, HIV, trauma, child protective services, and parental substance abuse.
  • Document and data entry is required, including case notes, assessments, emails, reports, and more.
  • Communicate with agencies, including the Housing Authority and Houston Coalition for the Homeless.
  • Complete required paperwork per Harris County procedures and HUD regulations.
  • Perform in-field work, including phone and/or virtual meetings, monthly home visits, phone and/or virtual visits with clients, landlord contact, and more.
Requirements:
  • Bachelor's degree in Social Work or related field.
  • Two (2) years of experience working with vulnerable populations or the homeless.
  • Valid Driver's License (Texas upon hire).
  • Ability to work with the homeless and engage clients with compassion.
  • Knowledge of the NASW Code of Ethics and the Fair Housing Act.
Preferred Qualifications:
  • One (1) year of experience working in the Child Protective Services sector.
  • One (1) year of experience working with foster children and/or families involved in the CPS system.
  • One (1) year of experience working with any HUD grant.
  • Trained in Trauma-Informed Care and Motivational Interviewing.
