About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our Case Management team at Memorial Hermann Health System. As a Case Manager, you will play a vital role in coordinating patient care, ensuring timely and effective transitions, and promoting high-quality outcomes.

Key Responsibilities
  • Coordinate patient care progression throughout the continuum, working collaboratively with physicians, nursing, and other multidisciplinary teams.
  • Address and resolve system problems impeding diagnostic or treatment progress, proactively identifying and resolving delays and obstacles to discharge.
  • Collaborate with the physician and multidisciplinary team to facilitate care for designated caseload, monitoring patient progress and intervening as necessary to ensure timely and effective care.
  • Facilitate the completion of diagnostic testing, treatment plans, and discharge plans, modifying plans as necessary to meet ongoing patient needs.
  • Communicate with third-party payors and other relevant stakeholders to ensure timely and effective care.
  • Assign appropriate levels of care, complete required documentation, and collaborate with medical staff, nursing staff, and ancillary staff to eliminate barriers to efficient delivery of care.
  • Collect and analyze data to drive decisions and implement performance improvement strategies related to case management, including fiscal, clinical, and patient satisfaction data.
Requirements
  • Graduate of an accredited school of professional nursing required; Bachelors of Nursing preferred, or graduate of an accredited Master of Social Work program.
  • Current and valid license to practice as a Registered Nurse in the state of Texas or current and valid license as a Master Social Worker (LMSW) in the state of Texas required, LCSW preferred.
  • Certification in Case Management required within two (2) years of hire into the Case Manager position.
  • Three (3) years of nursing or social work experience acute hospital-based preferred, or three (3) years of experience comparable clinical setting (i.e. ambulatory surgery center, infusion/dialysis clinic, Federally Qualified Health Clinic (FQHC), skilled nursing facility, or wound clinic).
  • Experience in utilization management, case management, discharge planning, or other cost/quality management program preferred.
