Key Responsibilities:
  • Assess patients' needs and develop comprehensive care plans to achieve optimal outcomes.
  • Collaborate with healthcare teams to coordinate patient care, including discharge planning and post-acute care arrangements.
  • Communicate effectively with patients, families, and healthcare providers to ensure timely and efficient care transitions.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Ensure compliance with regulatory requirements and hospital policies.
Requirements:
  • Graduate of an accredited nursing program.
  • Three years of hospital nursing experience, with a focus on case management or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in electronic medical records and other healthcare software.
Preferred Qualifications:
  • Case management certification (CCM or ACM).
  • Experience with utilization review and denial management.
  • Knowledge of Medicare, Medicaid, and managed care requirements.
