Pharmacy Technician

Job Summary:
As a Pharmacy Technician at LifeBridge Health, you will play a critical role in ensuring the accuracy and completeness of patient medication histories. Your primary responsibility will be to interview patients to obtain a comprehensive understanding of their medication regimens, including all current and active medications prescribed by their healthcare providers. You will also communicate with external sources to verify medication information and evaluate patient adherence to their medication regimens. Your attention to detail and ability to accurately document patient medication histories will be essential in supporting physician reconciliation and ensuring patient safety.
Key Responsibilities:
- Obtain accurate and comprehensive medication histories from patients through interviews and communication with external sources.
- Evaluate patient adherence to medication regimens and identify potential issues or concerns.
- Accurately document patient medication histories for physician reconciliation.
- Communicate effectively with healthcare providers and other stakeholders to ensure seamless patient care.
Requirements:
To be successful in this role, you will need to possess a formal working knowledge equivalent to an Associate's degree (2 years of college) and require knowledge of a specialized field. Additionally, you will need 3 years of pharmacy experience, with PTCB certification and Maryland Board of Pharmacy Technician registration, or be currently enrolled as a pharmacy or medical school student.
