Job Summary:
We are seeking a highly skilled Pharmacy Technician/Medication History Specialist to join our team at LifeBridge Health. As a Pharmacy Technician/Medication History Specialist, you will play a critical role in obtaining accurate and comprehensive medication histories for our patients.
Key Responsibilities:
- Interview patients to obtain complete medication histories
- Communicate with outside sources to determine all current and active medications prescribed to the patient
- Evaluate medication adherence based on patient interview
- Accurately document current medication history for physician reconciliation
Requirements:
- Formal working knowledge equivalent to an Associate's degree (2 years college)
- Requires knowledge of a specialized field
- 3 years of pharmacy experience, with PTCB Certified and Maryland Board of Pharmacy Technician Registration, or currently enrolled as pharmacy/medical school student
