Job Title: Registered Nurse - Interim Housing

At JWCH Institute, we are seeking a highly skilled and compassionate Registered Nurse to join our team in our Interim Housing program. As a Registered Nurse, you will play a vital role in providing medical care and support to homeless individuals, helping them improve their health conditions and enhance their social and activities of daily living (ADL) skills.

Job Summary:

The Interim Housing Registered Nurse will participate as a care team member of the Enhanced Services Program, which provides intensive case management services, behavioral healthcare, and offers a satellite medical clinic. This is a unique opportunity to make a meaningful difference in the lives of individuals experiencing homelessness.

Responsibilities:
  1. Conduct triage for all walk-in patients, including vital signs and complete assessments.
  2. Participate in all case conferences regarding ESP clients, supporting care teams in treatment plans.
  3. Complete TB screenings/readings and documentation.
  4. Administer oral, IM, SC, and SL medications as needed.
  5. Assist the Medical Providers with procedures as necessary.
  6. I.V. placement and monitoring, including the infusion of I.V. fluids as ordered by the Provider.
  7. Complete corrective action plans, if any, post internal/external audits.
  8. Monitor the medication management process to assist clients/patients in medication self-administration.
  9. Provide guidance and support to ESP staff regarding medical care, as needed.
  10. Take patient complaints and follow appropriate procedures in resolving any complaint.
  11. Provide patient education and teaching. Schedule patient hospital or doctors (specialist) appointment, if necessary.
  12. Assign daily nursing duties. Identify and correct patient flow deficiencies.
  13. Organize and maintain dispensary medication inventory, including removing expired medications and packaging them for proper disposal.
  14. Preparation and dispensing of all medications as ordered by the Provider, including the completion of dispensed medication labels.
  15. Place orders for medical supplies as needed.
  16. Collaborate with the Program Director and assist with the continuing improvement of systematic functions.
  17. Develop and monitor the communication network between the multi-disciplinary ESP staff and external professionals to improve continuity of care and to avoid conflicting information conveyed to clients/patients regarding their treatment plan.
  18. Ensure all PHI information is maintained in accordance with HIPAA regulations.
  19. Support program leadership in delivering all required services and meets outcome measures established by the Department of Health Services.
  20. Ensure quality measures, regulatory standards, and customer service standards are met.
  21. As needed, assist Program Director with reports on operational issues, organizational opportunities, and/or programmatic goals.
  22. Monitor and maintain adherence to the Nursing Training Manual and Agency Policies and Procedures.
  23. Perform other duties as required by the program and/or clinic.
Requirements:
  1. Licensed by the State of California to practice as a Registered Nurse.
  2. CPR Certificate by America Heart Association.
  3. At least one year experience in an ambulatory care setting.
  4. Experience working with patients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
  5. Experience working with chronically homeless individuals.
  6. Strong understanding of case management principles and practices.
  7. Ability to work collaboratively as a member of a multidisciplinary care team.
  8. Excellent communication and interpersonal skills.
  9. Proficiency in electronic health record (EHR) systems.
  10. Knowledge of and adherence to HIPAA regulations and patient confidentiality.
  11. Commitment to providing compassionate and culturally sensitive care to underserved populations.
  12. Familiarity with Housing for Health programs or similar initiatives is a plus.
  13. Bilingual (English/Spanish) preferred.
