At JWCH Institute, we are seeking a highly skilled and compassionate Registered Nurse to join our team in our Interim Housing program. As a Registered Nurse, you will play a vital role in providing medical care and support to homeless individuals, helping them improve their health conditions and enhance their social and activities of daily living (ADL) skills.
Job Summary:The Interim Housing Registered Nurse will participate as a care team member of the Enhanced Services Program, which provides intensive case management services, behavioral healthcare, and offers a satellite medical clinic. This is a unique opportunity to make a meaningful difference in the lives of individuals experiencing homelessness.
Responsibilities:- Conduct triage for all walk-in patients, including vital signs and complete assessments.
- Participate in all case conferences regarding ESP clients, supporting care teams in treatment plans.
- Complete TB screenings/readings and documentation.
- Administer oral, IM, SC, and SL medications as needed.
- Assist the Medical Providers with procedures as necessary.
- I.V. placement and monitoring, including the infusion of I.V. fluids as ordered by the Provider.
- Complete corrective action plans, if any, post internal/external audits.
- Monitor the medication management process to assist clients/patients in medication self-administration.
- Provide guidance and support to ESP staff regarding medical care, as needed.
- Take patient complaints and follow appropriate procedures in resolving any complaint.
- Provide patient education and teaching. Schedule patient hospital or doctors (specialist) appointment, if necessary.
- Assign daily nursing duties. Identify and correct patient flow deficiencies.
- Organize and maintain dispensary medication inventory, including removing expired medications and packaging them for proper disposal.
- Preparation and dispensing of all medications as ordered by the Provider, including the completion of dispensed medication labels.
- Place orders for medical supplies as needed.
- Collaborate with the Program Director and assist with the continuing improvement of systematic functions.
- Develop and monitor the communication network between the multi-disciplinary ESP staff and external professionals to improve continuity of care and to avoid conflicting information conveyed to clients/patients regarding their treatment plan.
- Ensure all PHI information is maintained in accordance with HIPAA regulations.
- Support program leadership in delivering all required services and meets outcome measures established by the Department of Health Services.
- Ensure quality measures, regulatory standards, and customer service standards are met.
- As needed, assist Program Director with reports on operational issues, organizational opportunities, and/or programmatic goals.
- Monitor and maintain adherence to the Nursing Training Manual and Agency Policies and Procedures.
- Perform other duties as required by the program and/or clinic.
- Licensed by the State of California to practice as a Registered Nurse.
- CPR Certificate by America Heart Association.
- At least one year experience in an ambulatory care setting.
- Experience working with patients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
- Experience working with chronically homeless individuals.
- Strong understanding of case management principles and practices.
- Ability to work collaboratively as a member of a multidisciplinary care team.
- Excellent communication and interpersonal skills.
- Proficiency in electronic health record (EHR) systems.
- Knowledge of and adherence to HIPAA regulations and patient confidentiality.
- Commitment to providing compassionate and culturally sensitive care to underserved populations.
- Familiarity with Housing for Health programs or similar initiatives is a plus.
- Bilingual (English/Spanish) preferred.
JWCH Institute, Inc., offers competitive salaries for all positions. Employees working 30 hours per week or more are provided a monthly allowance which can be used towards medical, dental, and vision premiums. Awardees receive up to $50,000 in exchange for two years of service if they practice in a HPSA of 14 above (Tier I).
