Job Overview
">We are seeking a highly skilled Nurse Practitioner (NP) to join our team as a HouseCalls Care Specialist. In this role, you will be responsible for conducting in-home assessments and providing high-quality care to patients in the comfort of their own homes.
About the Role
">- Conduct in-home assessments, including medical history evaluation, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses and develop care plans to address health risks and improve patient outcomes.
- Communicate findings with primary care physicians to inform care management and coordination.
- Educate patients on disease processes, medications, and compliance.
- Travel within Penobscot County to conduct home visits, with mileage reimbursement provided.
Requirements
">- Active NP license or ability to obtain by start date, with no encumbrances or disciplinary actions.
- Active national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or the ability to obtain certification by start date.
- Ability to complete physical demands of the job, including lifting up to 30 pounds and navigating various dwelling conditions.
- Reliable transportation for home visit assessments.
Compensation and Benefits
">This is a full-time position with a competitive salary range of $120,000 - $150,000 per year, depending on experience. We also offer a generous benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
