Job Overview
This PRN/Part Time position involves conducting medical risk assessments in various settings, including home visits, skilled facilities, and telehealth sessions. As a Medical Risk Assessment Specialist, you will work collaboratively with healthcare professionals to identify potential health risks and develop strategies to mitigate them.
Responsibilities
- Conduct thorough medical history reviews and assessments to identify potential health risks
- Develop and implement personalized care plans to address identified health risks
- Collaborate with healthcare professionals to ensure seamless communication and coordination of care
- Provide education and support to patients and their families to promote healthy lifestyle choices and disease management
Requirements
- Master's Degree in Nursing or Physician Assistant Studies required; OR commensurate experience and satisfactory completion of NP licensure
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner/Physician Assistant, with the ability to travel within the state or out of state
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
