We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at Matrix Medical Network. This is a PRN/Part Time opportunity for Home Risk Assessments in Bangor, Maine.
Job DescriptionIn this role, you will conduct comprehensive health assessments in the homes of our members, providing unmatched insight into their overall health and well-being. You will review medical history, medication adherence, social environment, and other risks, and provide recommendations for improved care.
This is a flexible schedule opportunity with days, evenings, nights, weekend, and flexible scheduling options available. As a valued member of our team, you will be rewarded with competitive compensation and opportunities for professional growth and development.
Responsibilities- Conduct home-based adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up services.
You will have the opportunity to work in various settings, including home visits, skilled facility visits, and telehealth visits.
Qualifications- A Master's Degree or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
- A minimum of 1 year of experience as a Nurse Practitioner/Physician Assistant, with strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills, with a positive and outgoing personality and a willingness to work as part of a team.
