Elevance Health is seeking a highly skilled LTSS Service Coordinator to join our team. As a key member of our care coordination team, you will play a critical role in ensuring that individuals with complex needs receive the support and services they require to live independently.
Key Responsibilities- Conduct face-to-face assessments with individuals to identify their needs and develop personalized care plans.
- Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
- Identify and address gaps in care, including potential clinical health care needs, and coordinate with the clinical healthcare management team to provide support.
- Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities, to ensure cost-effective and efficient utilization of long-term services and supports.
- Document short and long-term service and support goals in collaboration with the member's chosen care team.
- Identify members who would benefit from alternative levels of service or other waiver programs.
- Participate in process improvement initiatives and serve as a mentor, subject matter expert, or preceptor for new staff.
- Bachelor's degree in a healthcare-related field or equivalent experience.
- Minimum of 2 years of experience working with a social work agency or in a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to travel to worksites and other locations as necessary.
