Case Manager

The Wanda Alston Foundation is seeking a Case Manager to provide youth-centered, trauma-informed case management services for LGBTQIA2S+ individuals in transitional and extended transitional living programs.

Key Responsibilities:
  • Connect clients with medical and mental health services, public benefits, and community resources.
  • Develop and track individual service and treatment plans for each client.
  • Work closely with Housing Monitors to implement programming and assist clients in meeting their treatment goal plans.
  • Provide crisis intervention services and referrals for psychiatric assessment as needed.
  • Coordinate care with service providers, including medical, mental health, substance abuse, and other supportive services.
  • Provide on-call support and respond to emergencies.
  • Develop and maintain community partnerships to provide superior services.
  • Facilitate group life skills trainings and ongoing assessment and adjustment of services provided.
  • Accurately and timely document and report services through HMIS.
Requirements:
  • Bachelor's degree in Psychology, Social Work, or related field, plus a minimum of two years of relevant human service experience.
  • Current and active DC License required for candidates with a Master's degree in Counseling, Social Work, or Marriage & Family Therapy.
  • Experience with HMIS or comparable system and strong knowledge of case management documentation and record keeping.
  • Experience working in transitional living programs or similar housing programs.
  • Knowledge of substance abuse/recovery, family systems, medical and mental health treatment, and LGBTQIA2S+ issues.
  • Excellent interpersonal and written communication skills, as well as organizational and time management skills.
