Job Summary

We are seeking a skilled Case Manager to join our team at Hillcrest Children Center. As a Case Manager, you will play a critical role in providing home-based case management services to families who have recently experienced homelessness.

Key Responsibilities
  • Provide case management services to assigned participants, including housing location, family goal planning, and connection to community resources.
  • Facilitate and ensure connection to needed Community Services and work in collaboration with Community Agencies to ensure effective communication and participant engagement.
  • Connect assigned participants to and ensure their engagement in needed supportive services that will address barriers and challenges they face.
  • Track participant's engagement/participation in supportive services, workforce development, employment training, and other relevant activities.
  • Complete all required case notes, housing stabilization plans, documents, files, and assigned reports related to participants/case activity.
  • Develop and maintain individual participant's records/files that comply with all federal laws as well as DHS/FSA program standards.
  • Complete comprehensive monthly budgets with participants and input participant data and program activities into designated software database as assigned.
  • Prepare and issue all Notices to participants, including Exit, Termination, and Extension Notices.
  • Enter participant housing information into HMIS within 72 hours of lease-up.
Requirements
  • Bachelor's degree in social work, psychology, sociology, counseling, or related service/science or health care related disciplines.
  • Minimum of one year of case management experience.
  • Knowledge of community resources and excellent communication skills.
  • Ability to maintain confidentiality of records and information and adapt to changing needs of clients.
  • Ability to work with diverse personalities, persons of all ages, ethnicity, religion, and sexual orientation.
  • Proficiency in MS Office and excellent administrative skills.
  • Strong interpersonal and organization skills required, as well as strong communication skills, including verbal and writing.
  • Proven ability to submit timely, detailed, and accurate case notes.
