Job Title: Case Manager

Community Connections is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in providing supportive services to individuals and families in need.

Job Summary:

The Case Manager will be responsible for managing a caseload of up to 25 individuals or families, providing housing placement and community-based supportive services. This position requires a strong understanding of social services, mental health, and substance use disorders.

Key Responsibilities:
  • Develop and implement individualized service plans to meet the needs of clients
  • Provide direct case management services, including housing placement and supportive services
  • Collaborate with community partners to ensure seamless service delivery
  • Assess and refer clients to appropriate supportive services
  • Document all interactions with clients and community partners
Requirements:
  • Bachelor's degree in social work, psychology, sociology, or related field
  • 1 year of experience providing case management services
  • Experience working with adults with serious mental illness, chronically homeless adults, and/or adults with substance use
  • Excellent written and oral communication skills
  • Strong interpersonal and organizational skills
