Job Title: Case Manager

We are seeking a skilled Case Manager to join our team at Community Connections. As a Case Manager, you will play a critical role in delivering housing placement and community-based supportive services to individuals and families in need.

Job Summary:

The Case Manager will manage a caseload of up to 25 individual residents or families located in scattered sites or site-based locations in Washington, D.C. You will work with clients to develop service plans, set goals, and implement strategies to meet those goals under the direction of the PSHP Program Manager.

Key Responsibilities:
  • Develop and implement individualized housing stability plans for clients
  • Provide direct case management services to clients, including home visits, community visits, and phone calls
  • Assist clients in acquiring skills and resources necessary for successful community living
  • Assess and refer clients to appropriate supportive services to achieve service plan goals
  • Participate in psycho-educational and skill development groups as appropriate
  • Complete documentation within 48 hours of contact with consumer or network member
Requirements:
  • Bachelor's degree in social work, psychology, sociology, counseling, or related social service/science or healthcare discipline
  • 1 year's experience providing case management services
  • Experience working with adults with serious mental illness, chronically homeless adults, and/or adults with substance use
  • Excellent written and oral communication skills
  • Strong interpersonal and organizational skills
  • Ability to respond and remain calm in crisis situations
  • Ability to work independently and part of a small team
