Position Overview:

The Hospice Registered Nurse serves as the Case Manager for hospice patients, collaborating with the Interdisciplinary Team (IDG) to develop personalized care plans. This role involves organizing and directing services for patients, families, and caregivers based on assessed needs and goals.

The ideal candidate will have a strong nursing background with a focus on community health, along with experience in hospice, palliative, or end-of-life care.

Key Responsibilities

Patient Care:
  • Conduct initial assessments of patients and families to identify immediate needs and communicate them to the IDG.
  • Complete the HIS within two days of hospice election and again at discharge, adhering to hospice CoP's.
  • Perform comprehensive nursing assessments upon admission and election of hospice.
  • Coordinate with the IDG to ensure timely completion of all assessments within five days of hospice election.
  • Develop care plans in collaboration with the IDG, establishing goals based on the patient's diagnosis and needs.
Communication:
  • Prepare clinical documentation and communicate with the primary physician/hospice medical director biweekly.
  • Discuss patient needs with physicians and report any changes in condition.
  • Obtain and process physician orders as necessary.
  • Coordinate with the interdisciplinary team to update the client's care plan.
  • Ensure timely and compliant documentation as per hospice CoP's.
Qualifications:
  • Graduate from an accredited nursing program.
  • 1-2 years of recent nursing experience focused on patient-centered care.
  • Current state nursing license, CPR certification, and valid driver's license.
  • Bachelor's degree in nursing preferred, with hospice care experience.
  • Strong observational, communication, and problem-solving skills.
  • Physical ability to lift patients and equipment, with frequent walking, standing, reaching, bending, and kneeling.

Physical Requirements:
  • Prolonged periods of walking or standing.

Supervisory Responsibilities:
  • Oversee and coordinate care with LPNs and LNAs.
