Position Overview:
We are seeking a Senior Clinical Care Manager to join our dedicated team. This role reports directly to the Executive Director or Administrator.

Your primary responsibilities will include overseeing the daily operations of your designated hospice location, managing the clinical components of the hospice program, and ensuring the delivery of high-quality care to patients.

You will also supervise, manage, and guide the performance of assigned team members.

Key responsibilities include supervising and directing the work of specific team members, assigning admission and evaluation visits, scheduling appointments, and ensuring timely completion of visits. You will also manage the site in the absence of the Executive Director or Administrator.

Additionally, you will oversee clinical, administrative, and recordkeeping functions, respond to phone inquiries in a compassionate and professional manner, and participate in the on-call nursing schedule, providing direct skilled nursing care in patients' homes.

Candidate Profile:
**Education and Experience:** A degree in nursing is required.

Registered Nurse with an ASN or BSN from an accredited nursing school, with at least one year of experience as an RN (licensed in the state of employment).

Candidates should have a minimum of three years of direct patient care experience, one year of management experience, and two or more years in hospice or home care settings.

Must be proficient in using nursing assessment tools and office equipment, including computers and tablets. A deep sensitivity to the feelings of terminally ill patients and their families is essential.

Knowledge of the hospice concept and the needs of terminally ill patients and their families is required. Licenses, Certifications, and Registrations: A current nursing license in the state of employment, CPR certification, valid driver's license, and current automobile insurance are mandatory.

**Personal Traits and Skills:** Candidates should demonstrate flexibility by performing a variety of tasks, often shifting from one to another without losing efficiency or composure. Effective communication with individuals from diverse socioeconomic backgrounds is crucial. The ability to influence opinions and make sound judgments based on sensory or judgmental criteria is essential. Evidence of patient advocacy and responsible workplace behavior, along with maturity, cooperation, and tact in interactions with colleagues and hospice team members, is expected.


Specialized Knowledge and Skills:


In-depth knowledge of the needs of terminally ill patients and their families, as well as an understanding of the hospice concept, is required. Familiarity with the roles of various disciplines providing hospice services, excellent patient assessment skills, and strong oral and written communication abilities are essential. A thorough understanding of managed care principles and regulatory guidelines (e.g., Medicare, Medicaid) is also necessary.
