Role Overview:
The Hospice Registered Nurse serves as the Case Manager for hospice patients, collaborating with the Interdisciplinary Team (IDG) to develop personalized care plans. This role involves organizing and directing services for patients, families, and caregivers based on assessed needs and goals.

Key Responsibilities:
Patient Care:
  • Conduct initial assessments of patients and families to identify immediate needs, promptly communicating with the IDG.
  • Complete the Hospice Item Set (HIS) within two days of hospice election and again at discharge, adhering to the hospice Conditions of Participation.
  • Perform comprehensive nursing assessments upon admission and election of hospice services.
  • Coordinate with the IDG to ensure timely completion of all assessments within five days of hospice election.
  • Develop care plans in collaboration with the IDG, setting goals based on the patient's diagnosis and needs.
Communication:
  • Prepare clinical documentation and communicate with the primary physician or hospice medical director biweekly.
  • Report changes in patient conditions to the physician and obtain necessary orders.
  • Collaborate with the interdisciplinary team to update and coordinate the patient's care plan.
  • Ensure timely and compliant documentation in accordance with hospice regulations.
Qualifications:
  • Graduate from an accredited nursing program.
  • 1-2 years of recent nursing experience focused on patient-centered care.
  • Current state nursing license, CPR certification, and valid driver's license.
  • Bachelor's degree in nursing preferred, with hospice care experience being an advantage.
  • Strong skills in observation, communication, and problem-solving.
  • Physical ability to lift patients and equipment, with significant walking, standing, reaching, bending, and kneeling.

Physical Requirements:
  • Ability to engage in prolonged walking or standing.
  • Capability to lift, position, or transfer patients (up to 50 lbs.).
  • Frequent reaching, stooping, bending, kneeling, or crouching required.
  • Visual and auditory acuity necessary for nursing tasks.
Supervisory Duties:
  • Oversee and coordinate care with LPNs and LNAs.
