We are currently seeking qualified candidates for the Patient Referral Coordinator position. Please note: this role requires proficiency in both English and Spanish. This is a Regular, Full-Time, Non-Exempt position reporting to the Referral Supervisor, with a starting hourly rate range of $23.52 to $25.26 along with comprehensive benefits.
Primary Responsibilities:
The Patient Referral Coordinator is tasked with managing medical referrals to external healthcare facilities and ensuring appropriate follow-up. Key responsibilities include:
- Submitting and processing referrals electronically, via fax, and over the phone to external hospitals and clinics.
- Following up on referrals initiated by healthcare providers and through the referral office.
- Creating urgent referrals and managing follow-up as directed by healthcare providers.
- Reviewing referral requests for accuracy and completeness, and liaising with providers or nursing staff to gather necessary information.
- Informing patients about their scheduled appointments, including time, location, and any special instructions provided by the healthcare provider.
- Rescheduling referrals as necessary and communicating updates to providers.
- Documenting referral information accurately in the i2i Tracks log and maintaining records of follow-up actions.
- Ensuring timely receipt of reports through proactive follow-up with referral sources and specialists.
- Performing additional duties as assigned.
Qualifications:
- Bilingual in English and Spanish is required.
- High School Diploma or equivalent is necessary.
- Exceptional customer service skills are essential.
- Ability to engage effectively with individuals from diverse backgrounds.
- Familiarity with medical terminology is preferred.
