Job Overview

We are currently seeking qualified candidates for the Patient Referral Coordinator position. Please note: this role requires proficiency in both English and Spanish. This is a Regular, Full-Time, Non-Exempt position reporting to the Referral Supervisor, with a starting hourly rate range of $23.52 to $25.26 along with comprehensive benefits.

Primary Responsibilities:

The Patient Referral Coordinator is tasked with managing medical referrals to external healthcare facilities and ensuring appropriate follow-up. Key responsibilities include:

  1. Submitting and processing referrals electronically, via fax, and over the phone to external hospitals and clinics.
  2. Following up on referrals initiated by healthcare providers and through the referral office.
  3. Creating urgent referrals and managing follow-up as directed by healthcare providers.
  4. Reviewing referral requests for accuracy and completeness, and liaising with providers or nursing staff to gather necessary information.
  5. Informing patients about their scheduled appointments, including time, location, and any special instructions provided by the healthcare provider.
  6. Rescheduling referrals as necessary and communicating updates to providers.
  7. Documenting referral information accurately in the i2i Tracks log and maintaining records of follow-up actions.
  8. Ensuring timely receipt of reports through proactive follow-up with referral sources and specialists.
  9. Performing additional duties as assigned.

Qualifications:

  1. Bilingual in English and Spanish is required.
  2. High School Diploma or equivalent is necessary.
  3. Exceptional customer service skills are essential.
  4. Ability to engage effectively with individuals from diverse backgrounds.
  5. Familiarity with medical terminology is preferred.
