Job Overview

The Mental Health Association is dedicated to enhancing mental well-being by integrating physical healthcare to improve the treatment and support for individuals facing mental health challenges, while also combating the stigma surrounding mental health disorders and substance use issues.

MHA is currently seeking a full-time MICA Case Manager to contribute to our Partial Care - Prospect House initiative.

Qualifications:

  • Bachelor's degree in a relevant human services discipline.
  • A minimum of one year of pertinent experience is required.
  • At least two years of experience in delivering co-occurring services; a CADC or LCADC certification is preferred.
  • Proficiency in computer usage for electronic medical records management is essential.
  • A valid driver's license and the capability to operate a van are mandatory.
  • Experience in facilitating MICA group sessions is required.

Key Responsibilities:

  • Lead MICA group sessions.
  • Oversee the daily execution of the Partial Care Program in collaboration with staff, supervisors, and participants to provide essential services to individuals with severe and persistent mental health conditions, aimed at enhancing community integration.
  • Design and conduct milieu activities, including therapeutic, coping, and psycho-educational groups to foster social skills and promote relapse prevention and independent living.
  • Work closely with participants and staff to achieve psycho-social objectives and tasks of the unit.
  • Manage the milieu environment effectively.
  • Participate in unit outings, including driving responsibilities.
  • Deliver case management services for assigned participants, coordinating their medical, psychiatric, financial, residential, social, and legal needs.
  • Engage in regular goal-setting discussions with participants.
  • Provide crisis intervention as necessary.
  • Assist and advocate for participants in obtaining medical records and entitlements, particularly Medicaid/Medicare and food assistance.
  • Act as a liaison with families, boarding home personnel, and local agencies serving the elderly.
  • Consult frequently with program psychiatrists at Prospect House.
  • Organize transportation services and provide transport as required.
  • Collaborate with the Program Director and Supervisor to enhance the program, expand service offerings, select necessary supplies, and explore innovative program methods and activities.
  • Complete required in-service training.
  • Meet expected productivity standards regarding direct service delivery to consumers.
