Position Overview

VIA Health Partners is a distinguished leader in providing compassionate end-of-life care, recognized nationally for our commitment to community-based, non-profit hospice and palliative services. We are currently seeking a dedicated professional to fulfill the role of Nurse Preceptor.

Key Responsibilities:

  • Act as a mentor and preceptor for new nursing staff, facilitating their integration into hospice care settings.
  • Conduct clinical training visits and competency assessments to ensure high standards of patient care.
  • Provide guidance on nursing services and education, adhering to the regulations set by the North Carolina and South Carolina Boards of Nursing.
  • Collaborate with various service lines across regional offices to enhance the competency and performance of nursing staff.
  • Educate nurses on effective case management and the importance of interdisciplinary collaboration in patient care.
  • Evaluate the clinical skills and quality of care provided by new hires and existing nursing personnel.

Qualifications:

  • A diploma or associate degree in nursing is required; a Bachelor's degree is preferred.
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is essential.
  • Current CPR certification is required; CHPN certification is preferred.
  • A minimum of 2 years of direct patient care experience in hospice settings is required, along with at least 3 years of clinical experience as a Registered Nurse.
  • Proficiency in computer skills is necessary; experience with Homecare Homebase EMR is advantageous.
