Hospice Home Care Nurse

We are seeking a skilled Hospice Home Care Nurse to join our team at GBMC HealthCare. As a Hospice Home Care Nurse, you will be responsible for providing high-quality nursing care to terminally ill patients and their families in their homes.
Key Responsibilities:
- Identify patient/family care needs and provide nursing and supportive care
- Visit patients living in various facilities in the designated service area
- Collaborate with the healthcare team to develop and implement patient care plans
- Provide education and support to patients and families on end-of-life care
Requirements:
- Diploma, ADN, and/or BSN from an accredited institution
- Two years of nursing experience, or equivalent combination of education and experience
- Current Maryland State license as a Registered Nurse
- Certification in end-of-life care by the NBHPNA within three years of employment
Working Conditions:
- Normal office environment with driving as a large part of the day in the field
- Exposure to adverse working conditions due to performance of patient care activities and exposure to communicable diseases
Conditions of Employment:

- Valid Driver's License
