About the Role

We are seeking a highly skilled Primary Care Physician to join our team at CenterWell Senior Primary Care. As a key member of our care team, you will play a vital role in delivering high-quality, patient-centered care to our seniors.

Responsibilities
  • Evaluate and treat patients in accordance with established standards of care.
  • Monitor patient care using available data and chart reviews, ensuring adherence to quality and medical standards.
  • Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
  • Participate in daily huddles with the care team, contributing medical expertise and insights.
  • Collaborate with Regional Medical Director and Center Administrator to foster a culture of cooperation and professionalism.
  • Complete medical record documentation in a timely manner, working with quality-based coders to optimize coding specificity.
  • Adhere to policy and protocol defined by Clinical Leadership.
  • Meet with RMD to discuss quality of care, review outcome data, and address policy and record issues.
  • Participate in growth opportunities for new or existing services within the Center.
  • Participate in the local primary care 'on-call' program as needed.
  • Ensure personal compliance with licensing, certification, and accrediting bodies.
  • Focus 100% of time on direct patient care, including patient-facing time and administrative tasks related to patient care.
Requirements
  • Current and unrestricted medical license or willingness to obtain a medical license in the state of practice.
  • Graduate of an accredited MD or DO program from an accredited university.
  • Excellent verbal and written communication skills.
  • Demonstrate high-level interpersonal skills and ability to communicate effectively with colleagues and patients.
  • Fully engaged in the concept of 'Integrated team-based care' model.
  • Willingness and ability to learn and adapt to practice in a value-based care setting.
  • Superior patient/customer service skills.
  • Basic computer skills, including email and EMR.
Preferred Qualifications
  • Board Certification or eligibility to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active and unrestricted DEA license.
  • Medicare Provider Number.
  • Minimum of two to five years directly applicable experience.
  • Experience managing Medicare Advantage panels and understanding of best practices in coordinated care environments.
  • Knowledge of Medicare guidelines and coverage.
  • Bilingual skills a plus.
  • Knowledge of HEDIS quality indicators.
