Our unique care model focuses on personalized experiences, taking time to listen, learn and address the factors that impact patient well-being.
Our integrated care teams, which include physicians, nurses, behavioral health specialists and more, spend up to 50 percent more time with patients, providing compassionate, personalized care that brings better health outcomes.
About the RoleWe are seeking a highly skilled and compassionate Primary Care Physician to join our team at CenterWell Senior Primary Care. As a Primary Care Physician, you will be responsible for providing high-quality, patient-centered care to our senior patients.
You will work closely with our interdisciplinary care teams to develop and implement personalized care plans that address the unique needs and goals of each patient.
Responsibilities include:
- Conducting comprehensive physical exams and ordering diagnostic tests as needed
- Developing and implementing treatment plans to manage chronic conditions
- Providing education and support to patients and their families
- Collaborating with other healthcare professionals to coordinate care
To be successful in this role, you will need:
- A Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree
- Board certification in Family Medicine or Internal Medicine
- Current licensure to practice medicine in the state of [insert state]
